Westmoreland Care & Rehabilitation Center is seeking a caring LPN/LVN to join our nursing team in Westmoreland, TN. You will provide direct nursing care, administer medications, assist with ADLs, and supervise aides while upholding safety and dignity.

The role requires an active state license, BLS preferred, and at least 1 year of clinical nursing experience, ideally in long-term care. Strong documentation and EHR skills are essential for success.

Qualifikationen

• Active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory/charge nurse role preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Familiarity with EHR systems.

Aufgaben

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs) while ensuring dignity and safety.
• Supervise nursing assistants and medication aides during shifts.
• Monitor residents' status and report changes to physicians.
• Develop and review individualized care plans with the team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges per procedures.
• Participate in QA activities and regulatory surveys.
