Job summary

Signature HealthCARE Owensboro is seeking a dedicated LPN/LVN to provide skilled nursing care in a resident-focused environment. You will administer medications, monitor health status, and ensure comfort and safety for residents.

The role includes supervising nursing staff, maintaining precise documentation, and collaborating with the interdisciplinary team to update care plans. A strong clinical foundation, communication skills, and familiarity with EHR systems are essential.

Qualifications

• Active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Supervise nursing assistants and medication aides during shifts.
• Monitor residents’ status and notify physician of changes in condition.
• Develop, implement, and review individual care plans with the team.
• Document accurately in the facility’s records per policy.
• Coordinate admissions, transfers, and discharges following procedures.
• Participate in quality assurance and staff education activities.
