You will coordinate care plans, monitor residents, document changes, and communicate with families and the healthcare team while guiding new staff and responding to emergencies.

Qualificações

• Must hold a valid LPN license (state-qualified) and NCLEX-PN completion.
• Typical 1–2 years of nursing experience in long-term care.
• Strong leadership and organizational abilities are required.
• Excellent communication skills with patients, families, and staff.
• BLS certification is preferred or required depending on facility policy.
• Knowledge of state regulations and nursing home policies is essential.

Responsabilidades

• Supervise daily activities of nursing aides and other LPNs.
• Administer medications and monitor residents' vital signs.
• Develop and implement individualized care plans with the healthcare team.
• Maintain accurate and timely nursing documentation.
• Communicate care changes to residents, families, and staff.
• Assist in training and orienting new staff members.
• Respond to medical emergencies and coordinate with physicians.
• Participate in quality improvement initiatives within the facility.
• Manage medication orders and ensure proper administration.
• Ensure compliance with safety policies and regulatory requirements.
