Job summary

Henry Ford Health seeks a Supervisor for Integrated Case Management at our Rochester campus to lead daily case management operations and guide a team of professionals in assessment, planning, and care coordination. You will help ensure safe, effective, and cost-conscious care through collaboration with interdisciplinary teams.

The role emphasizes policy implementation, performance improvement, and advocacy for patient needs within a population health framework.

Qualifications

Bachelor's degree in nursing or related field (e.g., social work, counseling, health education) or Master's Degree in Social Work.
Three (3) years of clinical experience in case management, disease management, health coaching, nursing, or home health care.
Strong communication, interpersonal, analytical, and problem-solving skills.
Registered Nurse (RN) or Licensed Social Worker (LMSW) with a valid, unrestricted State of Michigan license.
Preferred: CCM certification from the Commission for Case Management Certification (CCMC).

Responsibilities

Oversee daily case management operations.
Monitor and guide a team of professionals in assessment, planning, and care coordination.
Participate in policy and procedure development to enhance workflow efficiency.
Maintain performance improvement activities to drive quality and cost-effective outcomes.
