Henry Ford Health seeks a Supervisor for Integrated Case Management at our Rochester campus to lead daily case management operations and guide a team of professionals in assessment, planning, and care coordination. You will help ensure safe, effective, and cost-conscious care through collaboration with interdisciplinary teams.

The role emphasizes policy implementation, performance improvement, and advocacy for patient needs within a population health framework.

Formación

• Bachelor's degree in nursing or related field (e.g., social work, counseling, health education) or Master's Degree in Social Work.
• Three (3) years of clinical experience in case management, disease management, health coaching, nursing, or home health care.
• Strong communication, interpersonal, analytical, and problem-solving skills.
• Registered Nurse (RN) or Licensed Social Worker (LMSW) with a valid, unrestricted State of Michigan license.
• Preferred: CCM certification from the Commission for Case Management Certification (CCMC).
