Qualifications

• Bachelor's degree in Nursing or Master's degree in Social Work required.
• RN or LMSW license with a valid, unrestricted Michigan license.
• Excellent verbal and written communication, and strong interpersonal skills.
• Strong problem-solving, analytical and decision-making abilities.
• Proficiency with computer systems and healthcare software.

Responsibilities

• Collaborate with care teams to identify patient needs and develop individualized care plans.
• Coordinate services and resources to ensure safe transitions to the next level of care.
• Advocate for patients and families, addressing social determinants of health and connecting with community resources.
