Job summary

Henry Ford Health is seeking a Case Manager to join our patient-centered care team. You will collaborate with clinicians to assess patient needs, develop individualized care plans, and coordinate services to ensure safe transitions between care settings.

The role emphasizes advocacy for patients and families, addressing social determinants of health, and evaluating outcomes to improve safety and quality. Proficiency with healthcare IT and a Michigan licensed RN or LMSW is required.

Qualifications

• Bachelor's degree in Nursing or Master's degree in Social Work required.
• RN or LMSW license with a valid, unrestricted Michigan license.
• Excellent verbal and written communication, and strong interpersonal skills.
• Strong problem-solving, analytical and decision-making abilities.
• Proficiency with computer systems and healthcare software.

Required Qualifications

• Bachelor¿s degree in nursing or Master¿s degree in Social Work.
• Minimum 3 years of clinical experience.
• Registered Nurse (RN) or Licensed Social Worker (LMSW) with a valid, unrestricted State of Michigan license.
• Excellent verbal communication, written documentation, and interpersonal skills.
• Strong problem-solving, analytical, and decision-making abilities.
• Proficiency in computer skills and knowledge of healthcare systems.

Preferred Qualifications

• Certification in Case Management (CCM) or Accredited Case Manager (ACM).
• Experience in discharge planning, home health care, rehabilitative medicine, or managed care.
