Job summary

Unity Hospice in Sturgeon Bay is seeking a Primary Registered Nurse to serve as lead case manager for patients at home and in facilities. The role is full-time, 36 hours weekly, with a rotating on-call and holiday schedule, requiring autonomy and collaboration with an interdisciplinary team.

You’ll assess needs, adjust plans, administer medications, and support families through end-of-life care, while traveling within the Sturgeon Bay territory as needed.

Qualifications

• Current unrestricted RN licensure in Wisconsin.
• Minimum of 2 years RN experience required.
• Valid Wisconsin Driver’s License and reliable transportation.

Responsibilities

• Provide compassionate hospice care and case management.
• Assess patient and family needs and adjust care plans accordingly.
• Respond to urgent situations and initiate crisis care when needed.
• Collaborate with physicians and interdisciplinary team members.
• Educate and support families through the end-of-life journey.
• Administer medications and document care in the EMR.
• Delegate and guide LPNs and CNAs.
• Manage daily priorities and assist with admissions.
