Job summary

Unity Hospice WI is seeking a Primary Registered Nurse (RN) to lead case management for patients in home and facility settings in the Sturgeon Bay territory, with occasional travel outside the territory.

As a full-time RN, you will collaborate with float nurses and an interdisciplinary team to deliver autonomous, compassionate, patient-centered care while supporting your team during fluctuating caseloads. Wisconsin RN licensure and 2+ years of RN experience required.

Qualifications

• Wisconsin RN license required.
• Minimum of 2 years RN experience.
• Valid Wisconsin driver's license with reliable transportation and proof of auto insurance.

Responsibilities

• Provide compassionate hospice care and case management.
• Assess patient and family needs and adjust care plans.
• Collaborate with physicians and interdisciplinary team members.
• Educate and support families through end-of-life journey.
• Administer medications and document care in the EMR.
• Delegate and guide LPNs and CNAs.
• Manage daily priorities and assist with admissions.
