Job summary

Affinity Hospice is seeking a dedicated Registered Nurse/Case Manager to plan, coordinate and provide compassionate hospice care in patients' homes and communities across Canton, GA. You will lead the interdisciplinary team and tailor care plans to individual and family needs.

The ideal candidate holds an active GA RN license, with 2+ years in clinical, home health or hospice settings, and a valid driver’s license.

Qualifications

• Must hold an active RN license in the state of Georgia.
• Minimum of two years of nursing experience in clinical care, home health or hospice preferred.
• Valid driver's license and insured vehicle required.
• Strong observation, communication and documentation skills.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team and family.
• Develop and document patient care plans and ensure OSHA-compliant practices.
• Maintain continuity of high-quality patient care with thorough documentation.
• Monitor cases for third-party payer requirements and outcomes.
• Supervise hospice aides and licensed practical nurses as assigned.
