Job summary

Affinity Hospice Holdings, LLC is seeking a Registered Nurse/Case Manager for PRN home hospice care in the Stockbridge, GA area. The role involves planning, directing, and providing compassionate hospice services within patients' homes and communities, with emphasis on community health education.

Minimum requirements include RN licensure, CPR, and at least two years of RN experience in clinical, home health, or hospice settings. PRN hours and driving responsibilities apply.

Qualifications

• Graduate of an accredited nursing program (Diploma/Associate/Baccalaureate).
• Current State license as a Registered Nurse.
• Current CPR certification.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement/develop/document the patient plan of care.
• Provide care utilizing infection control measures that protect both the staff and the patient (OSHA).
• Assure continuity of quality patient care delivered with appropriate documentation.
• Monitor assigned cases to ensure compliance with third-party payer requirements.
• Demonstrate commitment, professional growth, and competency.
• Promote agency philosophy and administrative policies.
