Affinity Hospice Care Inc. is seeking a registered nurse/case manager to plan, organize, and deliver hospice care in home and community settings. The role emphasizes nursing expertise, community health education, and coordination with an interdisciplinary team.

The position is full-time (40 hours/week) based in Birmingham, AL, requiring state RN licensure, CPR, and at least two years of hospice/home health experience. A valid driver’s license and reliable vehicle are expected.

Qualificações

• Diploma, ADN or BSN from an accredited nursing program.
• Current State RN license.
• Current CPR certification.
• Minimum 2 years RN experience in clinical, home health or hospice setting.
• Licensed driver with insured vehicle and valid registration.
• Strong observation and communication skills.

Responsabilidades

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement and document the patient plan of care.
• Provide care using OSHA infection control measures.
• Ensure continuity of quality patient care with proper documentation.
• Monitor cases for third-party payer requirements.
• Supervise hospice aides and licensed practical nurses.
• Perform on-call duties as assigned.
