Job summary

Sequoia Hospice in the Bay Area seeks a Clinical Manager to oversee coordination, quality, and delivery of hospice patient care. Qualifications

• Current California RN license.
• 2+ years hospice experience.
• 2+ years of management or supervisory experience preferred.
• Strong clinical assessment and leadership skills.
• Valid driver’s license, insured vehicle, and CPR certification.
• Ability to work collaboratively with interdisciplinary teams.

Responsibilities

• Oversee coordination, quality, and delivery of hospice patient care.
• Supervise interdisciplinary team members and mentor clinicians.
• Coordinate case referrals, home visits, and care plans.
• Collaborate with Medical Director on eligibility and needs.
• Lead quarterly clinical record reviews and documentation.
