Job summary

Sequoia Hospice in Concord, CA, is seeking an experienced Hospice Clinical Manager (RN) to oversee coordination, quality, and delivery of hospice patient care services. The role supervises the interdisciplinary team and supports clinicians in providing patient-centered care, with travel to field as needed.

The Clinical Manager will guide care planning, participate in quarterly reviews, and ensure compliance with hospice standards.

Qualifications

• Current California RN license is required.
• Minimum 2 years of hospice experience is required.
• Minimum 2 years of management or supervisory experience is strongly preferred.

Responsibilities

• Receive, review, and coordinate patient referrals to determine hospice needs.
• Assign appropriate hospice personnel to cases and ensure care coordination.
• Lead interdisciplinary team case conferences and quality reviews.
• Mentor clinicians, develop care plans and goals, prioritize interventions.
• Support recruitment, onboarding, and staff training; ensure policy compliance.

This role ensures clinical excellence, supervises interdisciplinary team members, and supports clinicians in providing outstanding patient-centered care.Pay Range: $140,000 – $150,000 per yearLocation: Concord office + in the field as neededJob Type: Full-timeKey ResponsibilitiesReceive, review, and evaluate patient case referrals to determine hospice needs, including conducting home visits.Assign appropriate hospice personnel to each case and ensure proper care coordination.Collaborate with the Medical Director regarding hospice eligibility and patient needs.Perform ongoing case evaluations through home visits, interdisciplinary meetings, and record review.Guide and mentor clinicians to promote highquality, compliant hospice services.Assist clinicians in developing care plans, establishing goals, and prioritizing interventions.Lead and participate in interdisciplinary team case conferences.Conduct quarterly clinical record reviews and communicate findings to leadership.Support recruitment, interviewing, and onboarding of new clinical staff.Assist with planning and delivering continuing education, staff training, and in-service programs.Support policy and procedure development and implementation.Ensure compliance with professional standards, hospice regulations, and organizational policies.Assess community health needs and support program development to meet those needs.QualificationsCurrent California Registered Nurse (RN) License required.2+ years of hospice experience required.2+ years of management or supervisory experience strongly preferred.Strong clinical assessment, communication, and leadership skills.Comprehensive understanding of hospice philosophy, end-of-life care, and pain/symptom management.Valid driver’s license, reliable insured vehicle, and current CPR certification.Ability to work collaboratively with interdisciplinary teams and support clinicians in the field.Why Join Sequoia Hospice?Sequoia Hospice is part of Cornerstone Group, a nationwide network of 75+ home health and hospice agencies.
