Qualifications

• Licensed RN, PT, OT, SLP, or MSW (active state license)
• 2+ years home health experience required
• Supervisory or leadership experience preferred
• Strong knowledge of home health regulations & documentation

Responsibilities

• Manage clinical staff and daily operations in a home health setting
• Develop and oversee plans of care (POC) and patient outcomes
• Review referrals and assign clinicians based on patient needs
• Ensure compliance with CMS, Medicare, and agency standards
• Conduct chart audits, case reviews, and case conferences
• Collaborate with physicians and support care coordination
• Participate in QAPI and performance improvement programs
• Assist with hiring, training, and staff development
