Job summary

PACE Southeast Michigan is seeking a Registered Nurse Case Manager to oversee comprehensive nursing care for frail elders in a home-based PACE setting. You will assess needs, plan and implement care with a multidisciplinary team, and coach participants and caregivers to maximize independence.

The role requires current MI RN licensure (BSN preferred), strong leadership, and the ability to educate and supervise staff, with on-call duties as needed.

Qualifications

• Registered Nurse with current Michigan licensure.
• BSN preferred.
• Minimum 1 year of experience with frail or elderly population.
• Must have a valid driver's license and reliable transportation.

Responsibilities

• Assess participants’ needs and plan appropriate nursing care.
• Collaborate with participant, family, and multidisciplinary team on care plans.
• Lead and supervise licensed and non-professional staff in home care delivery.
• Administer medications and monitor responses as ordered; ensure safe home management.
• Maintain accurate records and participate in quality assurance activities.
• Engage in patient teaching using adult learning principles; address caregiver education.
