Job summary

PACE Southeast Michigan seeks a Registered Nurse Case Manager (RNCM) to oversee comprehensive nursing care for frail elders in the PACE SEMI program. The RNCM directs licensed and non-professional staff, coordinates with families and the multidisciplinary team, and ensures education and safety in care delivery.

Responsibilities include assessments, planning, implementing, evaluating care plans, medication administration, and maintaining CMS-aligned standards while fostering participant autonomy

Qualifications

• Must be a Registered Nurse with current Michigan licensure.
• BSN preferred.
• One year of experience with a frail or elderly population.
• Must have a valid driver’s license.

Responsibilities

• Assess participants’ needs and plan appropriate nursing care during Initial Intake and Re-Evaluation Assessments.
• Collaborate with participant, family and multidisciplinary team to develop the care plan.
• Provide teaching, supervision, and counseling to participants or caregivers.
• Administer medications and treatments as ordered and monitor responses.
• Lead licensed and non-professional staff in delivering home-based nursing care.
• Document outcomes and communicate progress to the care team.
