Job summary

PACE Southeast Michigan is seeking a Registered Nurse Case Manager (RNCM) to lead in-home care coordination for frail elders with complex needs. You will assess, plan, implement, and evaluate nursing care with a focus on autonomy and family involvement.

Responsibilities include collaborating with a multidisciplinary team, educating participants and caregivers, and supervising nursing staff. A Michigan RN license and driving are required; BLS and infection control training are expected.

Qualifications

• Must be a Registered Nurse with current Michigan licensure, BSN preferred.
• Must have at least 1 year of experience with frail elderly population.
• Annual in-service training including infection control, TB testing, safety training, and BLS.
• Must possess a valid Michigan driver’s license and maintain a good driving record.

Responsibilities

• Assess participants’ needs and plan appropriate nursing care during Initial Intake and Re-Evaluation Assessments.
• Collaborate with participant, family and multidisciplinary team to develop the care plan.
• Maximize functional capacity and encourage autonomy in all aspects of care.
• Supervise and counsel participant or caregiver regarding nursing care needs.
• Educate participants, caregivers or family using adult learning principles.
• Initiate preventive and rehabilitative procedures as appropriate.
• Administer medications and treatments as ordered and monitor responses.
• Notify medical personnel of changes in status and document outcomes with the team.
• Maintain accurate records in accordance with CSI policies; participate in data collection.
