Job summary

PACE Southeast Michigan seeks an experienced Registered Nurse Case Manager to coordinate nursing care for frail elders in the PACE program. You will assess needs, develop plans with families and a multidisciplinary team, and lead nursing staff in delivering comprehensive care.

Responsibilities include teaching families, supervising staff, and ensuring safe medication administration. Strong clinical competence and advocacy for participants are essential.

Qualifications

• Must hold current Michigan RN license.
• BSN preferred.
• Experience with frail/elderly populations is desirable.
• Valid driver's license and reliable transportation are required.
• Excellent communication and teaching abilities are essential.
• Ability to work independently and with a team.

Responsibilities

• Assess participants’ needs at intake and during re-evaluation.
• Collaborate with participant, family, and multidisciplinary team to plan care.
• Promote autonomy and functional capacity in daily activities.
• Educate, supervise, and counsel participant or caregiver on nursing care needs.
• Provide educational information using adult learning principles to participants and families.
• Initiate preventive/rehabilitative procedures as appropriate.
• Administer medications and treatments as ordered and monitor responses.
• Notify medical staff of status changes and document outcomes.
• Ensure safe medication administration and adherence to regimen.
• Maintain patient records per CSI policies and standards.
• Support data collection for PACE information and program quality.
