Job summary

Community LIFE is seeking a Home Care Supervisor in Tarentum, PA to provide hands-on leadership and oversee Community Care Aides. You will coordinate with the IDT and families to ensure high-quality in-home care and support for elderly participants.

The role requires several years of direct care and supervisory experience, PA licensure, and strong collaboration skills. A 7:30a-4p schedule with on-call rotation is offered along with a comprehensive benefits package.

Qualifications

• 5 years direct care experience with at least 1 year in frail elderly care
• 2 years of supervisory experience
• Valid PA Driver’s License required
• Valid PA COTA/L, PTA, CNA or LPN license required

Responsibilities

• Supervise Community Care Aides with effective communication and guidance
• Strengthen the relationship between Home Care and the IDT
• Provide hands-on leadership in all aspects of home care
• May conduct in-home visits for assessment and ongoing case management
