Job summary

Community LIFE is seeking a Home Care Supervisor in Homestead, PA to provide leadership to Community Care Aides and to liaise with the IDT. The role entails on-site supervision, caregiver coaching, and ensuring quality home-based care for elderly participants.

Responsibilities include coordinating with families, performing in-home assessments when needed, and participating in weekend/on-call rotations to ensure continuous care coverage. A strong background in geriatric care is required.

Qualifications

• Graduate of an accredited nursing program for LPN/CNA.
• Program approved by the National Board for Certification in Occupational Therapy.
• Physical Therapy Assistant program approved by the American Physical Therapy Association.

Responsibilities

• Provide consistent supervision to Community Care Aides and guidance to staff.
• Strengthen the relationship between the Home Care Department and the Inter Disciplinary Team (IDT).
• May conduct in-home visits for assessment and ongoing case management; participate in weekend/on-call rotation.
