Job summary

A dedicated home care service provider is seeking a Home Care Supervisor to lead and supervise Community Care Aides. The ideal candidate will manage in-home assessments and facilitate care for the elderly while collaborating with families and interdisciplinary teams. This role involves ensuring high-quality home care services, participating in weekend/on-call rotation, and fostering relationships within the healthcare community. Qualifications

• 5 years of direct care experience with at least 1 year of experience working with the frail and elderly population.
• 2 years of supervisory experience preferred.
• Valid Pennsylvania COTA/L, PTA, CNA, or LPN license and PA Driver’s License.

Responsibilities

• Provide supervision to Community Care Aides.
• Strengthen the relationship between Home Care Department and Inter Disciplinary Team.
• Develop partnerships with families and caregivers.
• Make in-home visits for assessment and on-going case management.

Currently offering a $2,500 sign on bonus for this position

Location: Lower Burrell, PA

Schedule: 7:30a-4p M-F with on call rotation + as needed based on participant care

Summary of Job Duties: The Home Care Supervisor (HCS) will provide consistent supervision to Community Care Aides using effective communication and guidance. This person will strengthen the relationship between the Home Care Department and the Inter Disciplinary Team (IDT). This person will provide hands on leadership in all aspects of home care. The HCS will develop partnerships with families and caregivers. The HCS may be required to make in-home visits as necessary for assessment and on-going case management. This position will participate in weekend/on-call rotation.

Required Education

• Accredited nursing program for LPN or CNA.
• Program approved by the National Board for Certification in Occupational Therapy
• Physical Therapy Assistant program approved by the American Physical Therapy Association.

Required Experience

5 years of direct care experience with at least 1 year of experience working with the frail and elderly population. 2 years of supervisory experience. Experience teaching patients and other health care workers preferred.

Required Certifications/Licensure

• Interest in geriatrics and community-based programming.
