Job summary

WelbeHealth is seeking a Home Care Coordinator to perform in-home care assessments and coordinate personal care for participants in the program.

You will report to the Home Care Manager and work with a multidisciplinary team to ensure timely care delivery and appropriate scheduling. The role includes collaboration with nursing, therapy, social services, and physicians, plus involvement in end-of-life support.

Qualifications

• Healthcare licensure or equivalent (3+ years of professional experience).
• 3+ years of case management or nursing experience in a clinical or home setting with a frail or elderly population.
• Nursing knowledge and training to treat frail, elderly participants with complex conditions preferred.

Responsibilities

• Coordinate incoming calls related to participants, physicians, and agency services regarding orders, questions, and referrals.
• Communicate with participants by phone and coordinate with nursing, therapy, aides, social services, and physicians regarding schedule changes and updates.
• Track and monitor home care delivery and scheduling hours with Home Care Services staff.
• Partner with Marketing to support enrollment of prospective participants into the program.
• Assist with staffing and scheduling activities, soliciting, and manager input.
• Participate in end-of-life care, coordination, and support.
