Job summary

WelbeHealth is seeking a Home Care Coordinator to conduct in-home care assessments and set the framework for the home care team to help participants thrive. The role reports to the Home Care Manager and focuses on arranging, assessing, and overseeing personal care in participants’ homes.

The position offers a flexible hybrid schedule, allowing remote work with required onsite IDT meetings and home assessments, and a compensation package including base pay and bonus.

Qualifications

• Healthcare licensure or equivalent required.
• Minimum of three (3) years of case management or nursing experience in a clinical or home setting with a frail or elderly population.
• Bachelor’s degree preferred.

Responsibilities

• Coordinate incoming calls related to participants, physicians, and agency services.
• Communicate with participants by phone and coordinate with nursing, therapy, aides, social services, and physicians regarding schedule changes and updates.
• Track and monitor home care delivery and scheduling hours with the Home Care Services staff.
• Support enrollment of prospective participants into the program with the Marketing team.
• Assist with staffing and scheduling activities, soliciting, and manager input.
• Participate in end-of-life care coordination and support.
