Job summary

PACE Center Stockton is seeking a Primary Care Physician to join its interdisciplinary care team in Stockton, CA. You will provide comprehensive medical care to PACE participants, collaborating with nursing, social work, rehabilitation, dietary, and other staff to update each participant's plan of care.

The role requires MD/DO with CA license and DEA registration; geriatric or complex population experience is preferred. Equal opportunity employer; competitive compensation and benefits.

Qualifications

• MD or DO degree and active California license.
• Board certification in Internal Medicine, Family Medicine, or Geriatric Medicine preferred.
• Current DEA registration.
• Experience with geriatric or medically complex populations preferred.
• Prior PACE program or interdisciplinary care model experience preferred.

Responsibilities

• Provide comprehensive primary medical care to a panel of PACE participants.
• Lead and collaborate with the interdisciplinary care team to develop plans of care.
• Coordinate care across settings, including center visits, home care, and referrals.
• Order, interpret tests, and manage medications; support supervision of clinicians.
• Maintain timely documentation in the EMR and participate in quality initiatives.
