Job summary

Total Care Therapy in Troy, OH is seeking an Occupational Therapist to evaluate residents in assisted and independent living settings and to develop individualized plans to improve independence in ADLs, cognition, and upper extremity function. You will deliver skilled therapy, assess equipment needs, and collaborate with a multidisciplinary team to ensure safe, effective care.

A state OT license and CPR/BLS certification are required, with a driving need and reliable transportation.

Qualifications

• Current State Occupational Therapist (OT) license.
• CPR/BLS certification.
• Valid driver's license, reliable transportation, and auto insurance.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and establish individualized occupational therapy plans of care.
• Provide skilled occupational therapy interventions to improve ADLs, IADLs, upper extremity function, cognition, balance, coordination, and functional independence.
• Assess and recommend adaptive equipment and environmental modifications.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on adaptive techniques, home safety, fall prevention, and energy conservation strategies.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
