Job summary

Public Health Management Corporation is seeking a Lead Case Manager for the Family Finding program. The role focuses on coordinating family finding efforts, training new hires, and maintaining thorough case documentation.

You will work closely with DHS/CUA partners and community professionals to connect youth with their families. The position requires a Bachelor’s degree in social work or related field and at least three years of experience in family finding.

Qualifications

• Knowledge of social services and family systems in child welfare.
• Ability to communicate effectively with diverse populations.
• Strong writing and documentation skills.
• Willingness to adjust work hours to client needs.
• Experience facilitating public meetings is a plus.

Responsibilities

• Communicate with DHS/CUA workers and families to gather background information.
• Attend hearings and meetings related to caseload and bring relevant reports.
• Identify and contact numerous family members for each case and document outreach.
• Conduct field visits and plan for ongoing supports and next steps with professionals.
• Maintain up-to-date contacts and comprehensive case documentation.
