Adelphoi USA, Inc. is seeking a Case Manager I for the Family Finding program in Northumberland County, PA. This full-time role focuses on identifying and linking youth with permanent supports and connections within their family and community.

Responsibilities include conducting casework sessions, assessing safety and support needs, maintaining regular contact with youth and referring agencies, and documenting the process from Discovery to connections.

Qualifications

• BA/BS in Social Work or a related Human Service field.
• Minimum of 2 years of experience in the childcare field.

Responsibilities

• Searching for connections for the youth.
• Facilitating casework sessions to assess for support, and safety needs of youth.
• Maintaining regular contact with the youth, their support resources and the referring agency.
• Documenting the process from Discovery to finding supports and connections.
