Job summary

Hope Network in Holt, MI is seeking a Lead Direct Care Professional to provide oversight, support, and direct care to individuals served. You will lead shift, model person-centered support, promote independence, and ensure implementation of medical and behavioral plans.

This role also mentors staff, supports on-call coverage, and helps create a safe, respectful environment for those we serve.

Qualifications

• High school diploma or equivalent is required.
• Valid driver’s license.
• Ability to lift 50 lbs.
• Ability to pass background checks and fingerprints.
• CPR/First Aid certification or ability to obtain certification.

Responsibilities

• Assist with daily living (ADLs), personal care, and medications.
• Support community outings, transportation, and activities.
• Build positive relationships and encourage independence.
• Help maintain a safe, structured, and supportive environment.
• On Call rotation.
