Job summary

Qualifications

• High school diploma or GED is required.
• CPR/First Aid and Medication Management certification within 6 months of starting employment.
• Effective written and verbal communication skills.
• Ability to instruct persons with disabilities.
• Ability to adjust to unexpected situations.
• Computer proficiency.
• Reliable cellphone.
• Ability to handle confidential information.
• Loyalty to the agency and its mission and policies.
• Reliable transportation, valid driver’s license, and automobile insurance, required.

Responsibilities

• Acts as primary contact for parents/guardians of program participants at assigned site, county workers, and medical appointments.
• Monitors schedules and cleanliness of the site, and time off for program participants.
• Drives participants to appointments, events, stores, and activities.
• Maintains and reviews documentation for the site and submits it timely to the Program Manager.
• Assists with training new Team Members at the site.
• Supervises completion of Individual Program Plans and required documentation.
• Maintains accurate documentation on participants and updates guidelines and systems.
• Submits documentation on participants in a timely manner.
• Builds rapport with participants through effective communication.
• Provides crisis intervention and complies with reporting requirements.
• Maintains professional relationships with participants, families, coworkers, and agencies.
• Assists participants with developing strategies and following through on responsibilities.
• Encourages participants to achieve goals by programming based on preferences and strengths.
• Provides information about community resources and helps with self-advocacy.
• Ensures access to resources and coordinates them as needed.
• Provides ongoing assessment of participants’ needs and capabilities.
• Completes all duties and models IMPACT values.
