Job summary

Qualifications

• High school diploma or GED required.
• Minimum 1 year related experience; CPR/First Aid and Medication Management certification within 6 months.
• Strong written and verbal communication skills.
• Ability to instruct persons with disabilities.
• Adaptability to unexpected situations.
• Computer proficiency.
• Reliable cellphone.

• Encourage participants to achieve goals based on preferences and strengths.
• Provide information about community resources and help participants advocate for themselves.
• Drive participants to appointments and activities.
• Ensure access to resources and coordinate services as needed.
• Assess participant needs; document appropriately.
• Build rapport using participants' communication styles.
• Maintain accurate documentation and timely submissions.
• Provide crisis intervention as needed and refer to support systems.
