Job summary

L.A. Care Health Plan is seeking a Lead Community Resource Center (CRC) to oversee daily workflow and mentor assigned staff. You will guide performance, develop cohesive teams, and coordinate effective referral processes to connect non-member cases with external resources.

Qualifications

• Minimum 4 years of experience in community events and leading projects focused on health care, public health, or health education.
• At least 6 months of experience leading projects or staff with multiple stakeholders; senior CRC Specialist experience preferred.
• Health care product experience including Medicaid/Medi-Cal, Covered California, or Medicare SNP.
• Experience working cross-functionally with Care Management, Social Services, Health Education, and clinical teams.
• Salesforce CRM experience is a plus.

Responsibilities

• Lead the work of assigned staff, oversee daily workflow, provide guidance and feedback to improve performance.
• Manage complex projects, update stakeholders, develop timelines, ensure timely deliverables and implementation.
• Mentor staff on special projects, monitor risks, and implement process improvements for member engagement and service quality.
• Oversee member-centric customer service and connect members to external resources and programs.
• Support extended-hour operations and escalate issues to management as needed.
• Develop desk-level procedures and coordinate cross-functional health access initiatives.
