Job summary

L.A. Care Health Plan is seeking a Community Resource Center Specialist II for our South LA onsite location. You will build relationships with community partners and L.A.

Care teams to connect members with health resources and education programs, and support referral workflows to ensure positive health outcomes. The role emphasizes high-touch customer service, cross-department collaboration, and outreach activities to increase member engagement at the CRC sites.

Qualifications

• At least 6 months of community work focused on health care, public health, or health education, or related operations/coordination.
• Experience with data entry and reporting processes.
• Bilingual in one of L.A. Care's threshold languages is highly desirable.

Responsibilities

• Coordinate member-centric customer service to respond and resolve inquiries; bridge members to resources.
• Inform, guide, and educate members about health education programs, benefits, and CRC resources.
• Collaborate to create resource guides and reference materials for internal teams.
• Manage CRC calendar, conference rooms, and support services; assist with vendor coordination.
• Provide ongoing member assistance and follow-up; support outreach and engagement efforts.
