A Lead Community Health Worker (Lead CHW) is a seasoned, frontline public health worker who is a trusted member of and/or has a remarkable understanding of the community served. Under general supervision, provides day-to-day guidance to a team of Community Health Workers, who are working to improve health outcomes and eliminate racial, ethnic, and economic disparities in those outcomes. In this role, the Lead CHW will serve as a coach and mentor to Community Health Workers and ensure compliance with program guidelines. Acts as a link between the Community Health Workers, health system teams, and the community to facilitate access to services and improve the quality and cultural proficiency of service delivery.

Description:

Under general supervision, provides day-to-day guidance to a team of Community Health Workers, who are working to improve health outcomes and eliminate racial, ethnic, and economic disparities in those outcomes. In this role, the Lead CHW will serve as a coach and mentor to Community Health Workers and ensure compliance with program guidelines. Acts as a link between the Community Health Workers, health system teams, and the community to facilitate access to services and improve the quality and cultural proficiency of service delivery.

Essential Functions
  1. Supports the day-to-day functions of assigned CHW team; provides manager with updates regarding CHWs’ daily responsibilities, performance, and productivity, as well as internal resource needs.
  2. Assists with assigning incoming referrals and monitoring volume of referrals of assigned CHWs.
  3. Ensures CHWs follow protocols (e.g., patient encounters, documentation, etc.) with accuracy and in a timely fashion.
  4. Serves a limited caseload in a similar manner as CHWs, including providing basic education regarding chronic health conditions and health promotion, facilitating connection to health care and support services, and serving as a liaison between providers and patients.
  5. Schedules and facilitates team huddles and regular check-ins with each CHW to provide feedback, instruction, and recommendations for ongoing colleague development.
  6. As needed, supports integrating and promoting CHW services within the health system and community through health system and community events, speaking engagements, interdepartmental meetings, in-services, and distribution of program materials.
  7. Works with the supervisors and clinical care teams to establish and/or refine protocols for Community Health Workers engaging individuals, completing the documentation, and working with community resources.
  8. Supports CHW program and service enhancements where needed to better support individuals and improve the quality-of-service delivery.
  9. Serve as a mentor and coach to Community Health Workers.
  10. Maintains a working knowledge of applicable Federal, State, and local laws and regulations, Trinity Health’s Organizational Integrity Program, Standards of Conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical, and professional behavior.
Minimum Qualifications
  1. Experience as a Community Health Worker required; Must obtain a Trinity Health or equivalent CHW certification within three months of hire.
  2. High school diploma or GED. Three to five (3-5) years of community involvement experience.
  3. Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to effectively interview patients/clients and accurately record medical information. Ability to effectively present information in one-on-one and small group situations to patients/clients, and other employees of the organization. Ability to effectively relate pertinent information with medical and social service professionals.
  4. Work requires ability to communicate orally and to hear or communicate with target population. Bi-lingual skills to work with patients in their native language are preferred; certification as a qualified medical interpreter is a plus.
  5. Work requires proofreading and checking documents for accuracy.
  6. Must be comfortable working and serving in a diverse and inclusive environment, and operating in a collaborative, shared leadership environment with clinical supervision where applicable.
  7. Must be able to travel to the patients' homes as needed.
