Under general supervision, provides day-to-day guidance to a team of Community Health Workers, who are working to improve health outcomes and eliminate racial, ethnic, and economic disparities in those outcomes. In this role, the Lead CHW will serve as a coach and mentor to Community Health Workers and ensure compliance with program guidelines. Acts as a link between the Community Health Workers, health system teams, and the community to facilitate access to services and improve the quality and cultural proficiency of service delivery.
