Comagine Health is seeking a Lead, Clinical Care Management to oversee Oregon-based care management activities. In this role, you will conduct IQA assessments and develop person-centered service plans, collaborating with providers and community resources to ensure appropriate services.

You will also provide clinical leadership and training support to the Behavioral Health Case Manager team, guiding day-to-day assignments and case consultations.

Qualifications

• QMHP status is required.
• Graduate degree or nursing/OT licensure with behavioral health training.

Responsabilités

• Conduct assessments and develop PCSPs.
• Coordinate care and access to services.
• Provide team leadership and clinical support to the Behavioral Health Case Manager team.
