Qualifikationen

• QMHP status is required.
• Graduate degree in social work, psychology, or another behavioral health science field; or licensing in nursing or occupational therapy with relevant training.
• QMHP status must be achieved as part of the role.

• Conduct assessments and develop person-centered service plans (PCSPs).
• Coordinate care with providers, facilities, and community services; perform prior authorizations and eligibility determinations.
• Provide leadership and clinical support to the Behavioral Health Case Manager team; train and mentor colleagues.
• Ensure services are delivered per PCSP and address service concerns with stakeholders.
