Job summary

Qualifications

• Bachelor’s or Master’s degree in social work or related field.
• Minimum five years of case management experience; supervisory experience preferred.

Responsibilities

• Directs the Choice Case Management and Relocation Team and manages own caseload.
• Develops individualized care plans and coordinates services with partners.
• Tracks referrals, outcomes, and program metrics per HUD requirements.
• Partners with community agencies to secure housing resources and support services.
• Maintains program records and ensures compliance with regulatory standards.
• Leads team meetings and participates in quarterly participant updates.
