Vitality is seeking a licensed practical nurse to lead and support a wellness team in a senior community in Louisville, KY. You will ensure safe medication administration, monitor resident health, and coordinate with physicians to implement orders.

The role emphasizes compassionate care, compliance with state regulations, and supervision of Personal Care Specialists. CPR certification and active LPN/LVN license are required, along with strong communication in English.

Qualifications

Must be 21 years or older.
Strong English communication skills.
Meet state health related requirements.
Maintain current LPN/LVN licensure and CPR certification.

Responsabilités

Administer or assist residents to self-administer medications and treatments.
Measure vital signs and weight.
Communicate with physicians via telephone, fax, and Physician Communication forms.
Document pertinent information in Nursing Notes and resident charts.
Supervise Wellness team and Personal Care Specialists to ensure daily duties.
Attend onboarding, annual and in-service training and team meetings.
