Job summary

Vitality is seeking a licensed practical nurse/licensed vocational nurse to support resident care. You will administer medications, monitor vital signs, and help develop care plans under Wellness Director supervision.

The role requires CPR certification and current LPN/LVN license, with strong communication skills and compliance with state health regulations. Excellent teamwork and patient-centered approach are essential, in a fast-paced community setting.

Qualifications

• Must be at least 21 years old.
• Demonstrate ability to communicate effectively in English, both verbally and in writing.
• Meet state or provincial health related requirements.
• Maintain current professional license if LPN/LVN.
• Maintain current CPR certification if LPN/LVN.
• Maintain any other certifications as required by state or provincial regulations.

Responsibilities

• Administer or assist residents to self-administer medications and treatments.
• Measure vital signs and weight.
• Assist in development and revision of service plans and inform Wellness Director of resident needs.
• Supervise Personal Care Specialists to ensure daily completion of duties.
• Read shift change notes and communication logs, and report as needed.
• Attend onboarding and in-service training and team meetings.
