Job summary

The Methodist Home of Kentucky is seeking a Targeted Case Manager (TCM) to assess client needs, develop service plans, and coordinate resources within Medicaid guidelines. You will manage an assigned caseload and coordinate with community providers to ensure holistic support.

Required qualifications include a Bachelor’s degree in a human services field and completion of specific targeted case management trainings. A valid driver’s license and background checks are required to join the team.

Qualifications

• Bachelor’s degree in a human services field with at least 1 year of behavioral health experience post-bachelor’s
• Must complete DBH training requirements (12h TCM Core, 6h SUD TCM, 6h SED TCM, 6h SMI TCM)
• Must demonstrate strong communication and engagement skills
• Must be able to manage an assigned caseload and coordinate services within Medicaid standards
• Valid driver’s license and reliable transportation; background checks and drug screen required

Job description

Summary of Function

The Methodist Home is seeking a Targeted Case Manager (TCM) who is highly motivated to make a difference in the community and committed to the well-being of their clients. The TCM will utilize experience and knowledge to assess needs of clients and their families in order to link and coordinate the appropriate resources. They are responsible for managing an assigned caseload and responding appropriately to client needs in compliance with Medicaid standards. This includes but is not limited to: assessing client needs; developing, implementing and reviewing service plans; and working with other community resources in achieving client service needs. Caseloads may include diverse populations with a variety of clinical needs. The Targeted Case Manager is a positive team player that promotes a culture of inclusion and participation.

Education & Professional Requirements:

• Bachelor’s Degree from an accredited institution in a human services field with a minimum of 1 year of experience in a behavioral health setting post-bachelor’s degree.
• Certification in the following Department for Behavioral Health, Developmental and Intellectual Disabilities approved trainings to meet the requirements outlined in 908 KAR 2:260:
• Twelve (12) hour Targeted Case Management Core Training
• Six (6) hour Substance Use Disorder Targeted Case Management Training
• Six (6) hour Severe Emotional Disability Targeted Case Management Training
• Six (6) hour Severe Mental Illness Targeted Case Management Training
