Job summary

LifeLong Medical Care in California is seeking a Case Manager II to join our primary care team. This role conducts outreach, psychosocial assessment, and helps create patient-centered care plans, often working in patients' homes, clinics, or community settings.

The CM II leads Enhanced Care Management implementation, coordinates referrals, collaborates with the care team, and advocates for patients to access benefits and housing.

Qualifications

• Experience coordinating services for patients with complex health and social needs.
• Ability to conduct outreach, assess needs, and develop care plans.
• Experience with home visits and community-based settings.

Responsibilities

• Outreach to patients meeting program criteria and those prioritized for ECM.
• Engage with patients to assess strengths and needs and develop care plans.
• Coordinate referrals to internal and external resources and assist with applications.
• Maintain patient caseload per program standards and site requirements.
• Utilize data registries and reports to manage caseload and grant deliverables.
• Provide health education and harm reduction strategies to empower patients.
• Assist with accessing public benefits and housing.
• Communicate with care team, partners, and social supports.
• Document patient contacts in EHR/HMIS per policy.
• Represent LifeLong positively in the community.
