Job summary

Community Health Project Los Angeles (CHPLA) is hiring a full-time LEAD Case Manager to provide field-based intensive case management for CHPLA harm-reduction participants in Los Angeles. You will enroll participants, meet with them regularly, assess needs, and coordinate access to housing, health care, MAT services, and supportive resources.

The role emphasizes a trauma-informed, client-centered approach, requires a California driver’s license and reliable transportation for field work, and

Qualifications

• Experience linking people to housing, health care, entitlements, and treatment.
• Experience or fieldwork serving people who use drugs, people at-risk for overdose, and/or vulnerable populations.
• Experience writing curricula preferred.

Responsibilities

• Enroll participants and meet with them regularly to assess needs and link to services.
• Assess immediate needs at intake, including medical care, shelter, food, and harm reduction supplies.
• Develop individualized assessments and care plans addressing housing, health, and employment.
• Maintain client files and timely CHAMP data entry within 24-48 hours.
• Coordinate with partners across housing, health care, MAT, and legal services.
• Attend meetings and participate in case conferences and supervision.
