Role Description

The Lead Case Manager is a full-time, on-site role located in Warrensville Heights, OH. This role oversees case management services for program participants, ensuring individualized support plans are created, implemented, and monitored. The Lead Case Manager conducts intake assessments, maintains accurate case records, coordinates referrals to internal and external resources, and tracks client progress toward goals such as education, personal development, and community engagement. The position includes supervising or mentoring other case managers or volunteers, collaborating with program staff to align services with organizational mission, and participating in team meetings and community outreach activities.
