Qualifications

• Good communication with clients and team.
• Ability to follow verbal and written assignments.
• Proficient with ClearCare management system.

Responsibilities

• Assist with Activities of Daily Living (ADLs) per client plan of care.
• Assist with medication, housekeeping, meals, transportation, and companionship.
• Report daily activity to supervisor and maintain logs.
• Perform other tasks as outlined in the care plan.
