Avenues Home Care in Nashville is looking for an Experienced Caregiver to support elderly and vulnerable clients in the local area. You will assist with ADLs, meal prep, medication reminders, and mobility support, while documenting activities and reporting changes to your supervisor.

Required CNA or PCA license, CPR & First Aid, negative TB test, and reliable transportation. Strong communication and compassionate care are essential for success in this role.

Qualifications

• Must have strong communication skills with clients and families.
• Ability to follow verbal and written care plans to support ADLs.
• Proficiency with the Homecare Company Management System (ClearCare).

Responsibilities

• Assist clients with Activities of Daily Living (ADLs) as per care plans.
• Assist with medication reminders, light housekeeping, meals, and transportation as needed.
• Provide companionship and report changes in condition to supervisor.
• Maintain accurate daily logs and document activities per policy.
