Job summary

Benchmark Senior Living is actively seeking a Care Nurse Supervisor (LPN) to oversee the Resident Care Department under the leadership of the Resident Care Director. The CNS will supervise resident care staff, perform assessments, and ensure plans of care comply with state regulations and Benchmark standards.

The role requires coordination with providers, management of vital signs documentation, audits, and incident reporting, with a focus on resident safety and quality of life in the community.

Responsibilities

• Demonstrates expertise in clinical skills and nursing judgement within their scope of practice.
• Identifies and assesses the clinical status of potential residents.
• Obtains the medical information release form from primary care physician as needed.
• Develops a plan of care with the RCD based on residents’ problems, goals and planned interventions and implements it.
• Develops a service plan with the RCD according to state regulations.
• Coordinates care needs with community providers via a case management process.
• Measures and records vital signs (temperature, pulse, respiration, blood pressure, weight, height) as per state regulations.
• Oversees ancillary health services as needed.
• Provides ongoing supervision of residents’ health activities and seeks medical attention when required.
• Supports elder abuse prevention and reports per state regulations.
• Conducts SAMM / MAR and RCA documentation audits bi-monthly.
• Conducts assessments / reassessments per Benchmark standards.
• Assesses risk proactively.
• Facilitates weekly resident tracking meetings in the absence of the RCD.
• Ensures resident information is communicated to staff via reports and calendars.
• Communicates and documents residents’ material needs (e.g., briefs, clothing, toothpaste, pads).
• Notifies the RCD of incidents promptly and completes incident reports by day end.
• Observes and reports significant changes in resident behavior and health to the RCD, physician, family, RCAs, and Executive Director.
• Responds to emergencies and the personal emergency response system and knows when to call for backup.
