Job summary

Axis Community Health, Pleasanton, CA, is seeking a Complex Care Registered Nurse (RN) to support the Enhanced Care Management program in a Federally Qualified Health Center. The RN delivers clinical support, triage, and care coordination for high medical complexity patients, working with an integrated care team to promote health equity.

The role requires a BSN (or equivalent), 2+ years of experience with high-need patients, an active CA RN license, and proficiency with EPIC EHR and Axis

Qualifications

• BSN or equivalent required.
• Minimum 2 years experience with high-need/chronic conditions.
• CA RN license current.
• Knowledge of primary care/ambulatory care.
• CPR and First Aid as required.
• Ability to triage and make independent clinical decisions.
• Proficiency in HIPAA compliance and patient confidentiality.
• Strong communication and rapport-building with diverse populations.

Responsibilities

• Manage high-risk Medi-Cal caseloads with ECM team.
• Coordinate care across internal and external referrals.
• Provide RN coverage per Title 22 CC 75028.
• Adhere to infection-control and medication procedures.
• Document patient care in EHR and maintain continuity of care.
• Maintain HIPAA compliance and patient confidentiality.
• Collaborate with multidisciplinary team to meet patient needs.
