Qualifications

• 1+ years of general work experience, preferably in Case Management.
• Physically located in California.
• A valid driver’s license and auto liability insurance.

Responsibilities

• Maintain ongoing caseload using evidence-based approaches.
• Conduct periodic telephonic and SMS outreach for follow-up.
• Collaborate on care issues with the Enhanced Care Management team.
• Support members in securing connection to community supports.
