Job summary

Pair Team is seeking a highly motivated full-time Lead Care Manager to advance our whole-person, interdisciplinary care model. You will directly outreaching and engaging with individuals living with Serious Mental Illness, Substance Use Disorder, homelessness, and high medical needs, relying on trust and strong relationships to deliver care.

This position primarily enables remote work, with 2–3 on-site community visits monthly near your city.

Qualifications

• 1+ years of general work experience (Case Management preferred).
• Must be physically located in California.
• Bilingual English/Spanish required.
• Strong cultural fluency and empathy.
• Able to collaborate in a multidisciplinary team.
• Proficient with CRM databases, basic Excel, Word, email, and video conferencing.
• Valid driver’s license and auto liability insurance.

Responsibilities

• Maintain ongoing caseload of individuals through evidence-based approaches to promote engagement and health goals.
• Use relationship-based strategies to support members with social support navigation.
• Conduct periodic telephonic and SMS outreach to ensure timely follow-ups.
• Work with members to identify health/wellness goals and incorporate them into Health Action Plan/Shared Care Plan.
• Support nurse care manager, behavioral health care manager, nurse practitioner and Community Engagement Specialist with delegated tasks.
• Collaborate on care issues with Enhanced Care Management team in reviews and with clinicians before actions.
• Meet monthly encounter metrics to ensure compliance with health plan regulations.
• Identify and break down barriers ensuring continuation with the program.
• Assist individuals in securing connections to community supports and referrals.
• Coordinate physical care management appointments with providers.
• Utilize external and internal platforms to collaborate with team members and carry out daily tasks.
