Job summary

An innovative healthcare organization in California seeks a highly motivated full-time Lead Care Manager. This role focuses on engaging individuals with Serious Mental Illness/Substance Use Disorder, experiencing homelessness, and/or those with high medical needs. The position is primarily remote, with 2–3 on-site visits per month. Candidates should be bilingual in English/Spanish, possess strong empathy and organizational skills, and be comfortable with technology. Qualifications

• 1+ years of general work experience, ideally in Case Management.
• Located in California with reliable transportation.
• Valid driver’s license and auto liability insurance.

Responsibilities

• Maintain ongoing caseload of individuals with health goals.
• Conduct telephonic and SMS outreach for follow-ups.
• Collaborate on care issues with Enhanced Care Management team.
